We haven’t cruised in several years. Do they still do the champagne waterfall? What about the captains reception? What day is that usually held? Thank you!

Yes it's still being done usually on a formal night along with Captains reception for platinum and elites.

I *believe* (could be wrong) that it is always near the beginning of the cruise. Couldn't say for sure if it is always on a formal night but that would make sense.

I believe you're correct. This way it gives people a longer time to decide if they want to purchase the photo or not.
