Printable Medical Form

I remember seeing a link to a printable medical form to save a bit of time at embarkation but silly me forgot to save the link.

 

Would anyone happen to have it?

Is this accepted on all cruise lines?

I know it is accepted by RCL. Have not tried it on other lines but I would think it is accepted by all.

Don't be surprised if they make you fill out another one. They don't know if you filled it out the day of embarkation or 2 weeks prior.

We used this form on our recent cruise on Mariner of the Seas when we sailed on October 20th .

They did take the form off us but made us fill in another form at the pier.

Possibly because embarkation was delayed till 3pm due to deep cleaning due to a problem with Noro on the previous cruise.:eek:

Sounds like your just better off doing one, at the pier.

We just do ours at the pier. It just takes a second. Except last cruise, they gave us the questionares in Spanish. I have some Spanish, but not enough to be sure of the questions.

Right. You fill it out while talking to the check in agent. No need to do it ahead of time.
