Help with Open Booking Form

I just printed my open booking form to take with me on FOS. In the travel agent section it says "Vacation Planner USA". I've never used a travel agent and don't want to! Has anyone seen this and can I just cross it out when I fill out my form?

Did you use RCL's booking agent when you booked the original cruise?

 

Our cruise docs have ROYAL ONBOARD CRUISE SALES as the booking agent on the form, and all of our NCCs have my email name as the ATTN TO:

 

(we've always booked using the online booking engine)

I would wait and fill out the onboard form. The form in your documents has 2 names (if you are traveling as 2) and will require a $200 deposit. The onboard forms only need one name and $100.

 

Filling it out onboard will also resolve the issue of who it will be assigned to. You can make sure it is assigned to yourself.
