If it didn't impact me I wouldn't say a word. But it's not allowed and therefore I'm in the right to report them, especially when their unpermitted actions impact my enjoyment of my balcony. I couldn't care less about things that don't affect me personally, such as attire, but smoke makes me feel sick to my stomach and affects my overall health. If I paid the extra money to enjoy a balcony I would not hesitate to rat someone out for doing something that is actually ruining my trip. If someone were wearing a hat in the MDR, whatever. If you are smoking in the designated areas then I don't complain. I just try to avoid them as much as possible. It's one of the reasons I don't go to the casino.
