The Physician is named and their address on the result letter for Allure was that of the port we left from:

 

RCG Port Canaveral Terminal 1 Allure of the Seas RCG Por 9050 Discovery Rd.
Port Canaveral, FL 32920

 

Didn't seem to bother CBSA at the border who asked for it.
