Is there a form to use/fill out to send? I know there is one for Princess cruise line. What about for Carnival cruise line? If so where can I find it ?

There is no form.  I emailed a copy of our brokerage statement showing our Carnival stock with the account number blacked out to guestadmin@carnival.com.  In the email I requested the onboard credit and included our names, VIFP numbers, booking number, ship and cabin number.  I received an email reply three days later confirming they had applied the OBC to our upcoming cruise in April.  I sent the email in October.

Several posters have recently reported that Carnival will no longer accept this request via e-mail.  They are requesting that it be sent either via regular mail or fax.

They do prefer fax over email. I got a fax machine at Office Depot for $29.95. They always process it very quickly.
