Health Questioner

I am paid in full and printed out my boarding documents, but I don't see where the Health Questioner is.

From what I have been told, they now take your health information when you check in at the port.

I am on Sunshine out of Port Canaveral and the health questionnaire was removed from my documents and is no longer needed.

This. I sailed leas then a month ago, no physical form needed.

It's like 3-4 "yes or no" questions. Not a big deal to do it there.

It would seem it depends on the port of departure from the replies on this (and other) threads on the topic. In FLL two months ago, they did not ask for the forms or look at them (I had them in hand) or even ask any of the health related questions.

You check off a few boxes and sign your name. What makes it such a pain? It isn't a census survey.

And we were just on the Magic out of Port Canaveral a few weeks ago and didn't have to do the health form.
