Tracking the source would be very difficult. The form is rather silly IMO. Does anyone ever answer yes? The question should really read "do you want to be denied boarding without a refund? Yes/No"

Why do you ask this? Will it make the difference whether you take the cruise or not? If they have one fill it out if they don't then they don't. At the worst, it will take about 2 mins of your time before boarding.

Had to do it in February on Carnival an in January on Princess.

Been filling out these types of forms since the Swine Flu fiasco.

Well we used to have to fill them out at check in, so now

We had to do it in December. I was actually sick when we boarded but I had been sick for a while and had been on medication so I was no longer contagious (it was just a sinus infection). I wasn't honest on my questionnaire, I admit, but it was my very first cruise and I didn't want to be told to go home or something awful.

It prints out when you print your boarding pass.

We had them in January - printed out & completed at home, but they also had them at check-in in Galveston.

I have filled it out at the port while in line on all of my cruises since 2008....thought it was routine.

Nope. I was just asking to find out if I actually needed to print it out, in an effort to save paper and conserve this beautiful planet we call home. Relax.

I printed out our booking summary for June 3 2016 and the health form is there.

January 29 it was there. You had to turn it in before you cools check in

Remember when Ebola was all the rage. Now you never hear of it. I guess they cured it. Or maybe the news folks found something new to blow up all out of proportion. :rolleyes:

Just got off the Liberty and we still had to turn it in. Princess requires it - 2011 & 2014. But this was the 1st time on Carnival when they required it.
