Hotel Smeraldo - small boutique hotel near most everything.  Walking distance to Trevi Fountain, the Pantheon and the Coloseum. When we stayed breakfast was included. There is a fabulous bakery ($$) across the street.  Can't say how much we enjoyed this hotel.  Rooms are typically small, but great bathroom and room

There is a long thread on the Italy Ports of Call forum here on Rome hotels, with lots of input on various hotels around the city center/tourist area. 

 

 

The key is to be centrally located. If you are, it's easy to walk to just about everything, except possibly the Vatican which is outside of the "magic circle".

 

I've stayed in several hotels across the city on many visits, and my favorite remains the Albergo del Senato, located on the same piazza as the Pantheon.

+1! Loved it, great staff, quiet but charming. And they serve a prix fixe dinner nightly or almost and it's awesome!

We stayed recently at U Visionary near Trevi. It just opened in the last year or so. Very very dark rooms (which were great for sleeping off jet lag) and very large room and bathroom, especially by European standards. Breakfast was included on site. They will arrange any transportation you need. Would definitely stay here again.
