There are very few areas where you can smoke onboard to begin with, so as long as people are following the rules, I see no reason it should adversely affect non-smoking guests. While I would love to book a non-smoking cruise because I am a asthmatic, my cruise partner (mom) likes to smoke e-cigarettes so i wouldn't because it means she wouldn't go with me on our yearly mother-daughter trips. Really don't have a problem with smokers EXCEPT in the Casino. There should be a section just for the Smokers that keeps all the smoke in that section.
